Hi I don't know the full story for your film yet, but I just had a quick idea, instead of making the interviewee the crazy guy, a twist in the end could be that it's the psychiatrist who is crazy, and these are all personalities that he has made up, and his final personality is that of the psychiatrist. Just a quick thought, instead of an obvious twist.
